     Please print this form and mail it in with your donation check, or simply fill out the credit card
     donation section at the bottom of the form.

    1.  Decide if you want to download the donation form as a Microsoft Word or Adobe PDF file.

    2.  Click the appropriate button below to download the form to your computer.

    3.  When the download window pops up on your computer, you must click "SAVE" in order to save a
         copy of to your computer. 

    4.  Be sure and notice where on your computer you saved the file.  You will need to go to that folder
         and double-click on the document entitled, "NCSchoiceCommitment" to open and print it.
